As of 6/29/2019, we found out that SMW sees "Mad Anne" as a page, when it actually does not exists. This may be caused by the page "Beast Dungeon" creating a link, and internally, the page is now wanted. SMW sees this as a page and reports it as a real item, and then Template:Exists returns true (or 1) and causing many problems.
While I tried to find a difference between page does not exists vs does using #ask, my rabbit hole of #sub and #pos failed. The next thing I can thing of is inside the callers of Exists, such as Template:Item and Template:Monster Link, determine if file:Mad Anne.png exists and if there is none, report default No_icon.png.
